Aged and Disability Services

Central Goldfields Shire Council provides services to support residents to continue living in their own homes and to be actively involved in their community.

Services for people aged 65 and over (50 and over for Aboriginal and Torres Strait Islander people) are directly funded and managed through the Commonwealth Home Support Program (CHSP).

Services for people aged 65 and under are funded by the Victorian State Government under the HACC PYP Program. 

People aged under 65 with a disability may be eligible for the National Disability Insurance Scheme (NDIS). For more information visit the NDIS website. 

Assessment for Commonwealth Home Support Services (over 65)

People aged 65 and over (50 and over for Aboriginal and Torres Strait Islander people) must undergo a living at home assessment by the Regional Assessment Services.

Assessment for Home Support Services (under 65)

People aged under 65 (under 50 for Aboriginal and Torres Strait Islander people) must undergo a living at home assessment. To arrange an assessment, please contact our Assessment Team on 03 5461 0649.

Once a referral for services is received, an Assessment Officer will visit the home to discuss and determine the person’s needs and eligibility for services. If eligible, an individualised care plan will be agreed upon by both the client and the Assessment Officer, detailing the services and tasks required.

Reassessments for clients are conducted routinely, or when the household and/or client circumstances change.
